Voting booth

I use a CGI script called formail to run a football pool. I modified the html to make a little voting booth, if someone wants to use it for future votes.<p>
I made a sample that would look a bit like the last vote for banning some games. http://www.kaupp.cx/vote/

Normally the results are emailed to a maintainer. The message board method still works, I was just bored at lunch today.
